SASA Spirits Fall Program Evaluation

Parents, This survey will be used to evaluate our programs and operations. This information will be shared with the board and myself, as we look to continue providing a great SASA experience to Coaches/Players/Parents. This will be a confidential survey, entries will be identified by team and age group. If you have serious and immediate issues please feel free to contact a member of the SASA board or DOC. Please note if you have multiple players at SASA to do a survey for each player.
