There will be NO Friday or Saturday registration times. ALL REGISTRATION DOCUMENTS MUST BE UPLOADED BY MAY 21st, 2021

1. ALL TEAMS: 1.Current IYSA Risk Management Certification 2.  IYSA Communicable Disease Release of Liability and Assumption of Risk Agreement

2. IYSA TEAMS: IYSA Tournament Rosters & Guest Player Forms

3. NON-IYSA TEAMS:
• Valid State Association or US Soccer Affiliated Organization Rosters & Guest Player Forms
• Travel Permits
• IYSA Emergency Medical Release & Liability Waivers (“Waiver”) for players and coaches This form must not be altered
• IYSA Communicable Disease Release of Liability and assumption of Risk Agreement for players, coaches and associated Team personnel
• Form NT from Non-IYSA/USYS Teams
